Every player is required to submit a signed Elite Junior Classic Waiver which you can download below and a copy of the player’s physical completed within the last 12 months. We prefer that you upload both of these files during the online registration process, but if you did not do so at the time and now have the files ready to upload, please click on the “Submit Forms” button below.
If you do not have access to a scanner, please plan to bring the forms with you to the informational meeting on TBA at McEachern High School Cafeteria.
